Do I need an insurance referral?

Most managed care programs and Medicaid require a written referral from your primary care physician. Without this form, your insurance company will refuse to pay for your visit, and therefore you will be asked to pay for services at the time of your appointment, prior to being seen. We are required to comply with contractual rules of your health plan, so please help us by attending to your part of the paperwork.
